The Prevention Protocol: How to Taper Off an Aromatase Inhibitor

Abrupt cessation is the primary cause of E2 rebound. The clinical solution is a structured taper.

Week 1 (Taper Phase): Reduce your AI dose by 50%. If you were taking 200mg Tongkat Ali daily, drop to 100mg. If you were using pharmaceutical AIs, consult your prescribing physician for a specific taper schedule; this guide is intended for natural supplementation only.

  • Any lump, persistent tenderness, or swelling beneath the nipple tissue requires immediate medical assessment: do not attempt to self-manage with additional AIs
  • Joint pain severe enough to limit daily function should be assessed by a physician; extreme E2 crashes can cause stress fractures in high-volume athletes
  • Mood disturbance severe enough to affect work or relationships warrants bloodwork before continuing any hormonal supplementation
